A little about you...

As part of the new starter pack, we ask parents/guardians to complete 3 forms before their child starts so that we have everything we need for the first day.

Form 1 - Intake Form - this asks for your child's details including emergency contact numbers, medical needs and dietary requirements.
(In addition, if your child requires any medicines or asthma inhalers in school, we will need you to complete a healthcare plan or 'Medicine in School' form for your child's first day.  Please call the school office to arrange this.) PLEASE NOTE - THIS IS FOR PUPILS WHO HAVE ALREADY BEEN ALLOCATED A PLACE AT IVJS BY BUCKINGHAMSHIRE COUNCIL. TO APPLY FOR A SCHOOL PLACE, PLEASE VISIT WWW.BUCKINGHAMSHIRE.GOV.UK

Form 2  - Consent Information -this is to let us know what consent you give for your child during the school day.

Form 3 - Dolce Account - this asks your permission to set up a lunch account with our school for Dolce (our lunch provider).

We are very lucky to have an onsite kitchen. Our Diner is run by Dolce and each child can order lunch in class or from home . Remember to sign up for a Dolce account (see Form 3) and if you would like to check to see if you qualify for Free School Meals please contact the school (remember only Key stage 1 children are automatically provided with free lunches).
